Unlike older MAOIs, Emsam is the first and only transdermal MAOI approved by the FDA for the treatment of depression. Because it’s delivered via patch, it bypasses the digestive system, which may help reduce some of the dietary restrictions and gastrointestinal side effects typically associated with MAOIs.
What Emsam Treats
Emsam is primarily used to treat Major Depressive Disorder, especially in adults who have not found relief with other antidepressants. It may be considered when first-line SSRIs or SNRIs haven’t been effective or have caused intolerable side effects.

How Emsam Works

The patch is applied once a day, typically to the upper torso, upper thigh, or behind the ear. It’s important to rotate application sites to avoid skin irritation. The patch comes in different strengths, and your provider will determine the appropriate dose for you based on your needs and how you respond to the treatment.
What to Expect When Starting Emsam
Starting Emsam involves a careful conversation with your psychiatric provider, especially regarding medications, diet, and health history. One of the advantages of the lower-dose Emsam patch (6 mg/24 hours) is that it does not require dietary restrictions. However, at higher doses (9 mg or 12 mg), patients need to avoid foods high in tyramine, such as aged cheeses, cured meats, and certain wines, as these can cause dangerous increases in blood pressure when combined with MAOIs.
Early in treatment, some people may notice mild side effects including:
- Skin irritation at the patch site
- Insomnia
- Dizziness
- Mild nausea or headache
